What does a transplant kidney ultrasound assess?
A specialised Doppler examination of the graft — the kidney itself, sitting in its transplanted position in the iliac fossa, and the blood vessels that keep it alive. We assess the graft’s size and structure, look for fluid collections around it, check for any swelling of its drainage system, and — the heart of the examination — interrogate the arterial and venous blood flow, measuring resistive indices at multiple points through the kidney. These flow measurements are among the earliest windows into how a graft is coping.
Why is this scan referral-only?
Transplant patients come to us strictly referred by their consultant nephrologist or transplant team, as part of rigorous surveillance for blood-flow changes and early signs of rejection.
Self-referral is entirely inappropriate here — not as gatekeeping, but as clinical honesty. Interpreting the delicate haemodynamics and resistive indices of a graft is impossible without knowing your precise baseline and complex clinical history. A number that is normal for one transplant is an alarm bell for another; only your transplant team holds that context. Our role is to produce meticulous, comparable measurements. Theirs is to interpret them against your history.
You have pain or swelling over the graft, a sudden drop in urine output, fever, or feel suddenly unwell. Transplant units run 24-hour advice lines for exactly these moments — they are always your first call, and they will direct any imaging needed.

Why comparable measurements matter more than single numbers
Graft surveillance is a time series, not a snapshot. The value lies in consistency.
Resistive indices in context
A resistive index is meaningful only against the graft’s own history and the clinical picture. The same figure can be entirely normal or a reason to act, depending on whose kidney it is.
Protocol consistency
Measurements taken at the same levels, in the same way, each time are what make a trend visible. We scan to the protocol your unit specifies rather than our own preference.
