What Do the Red and Blue Colours on an Ultrasound Mean?

Not arteries and veins. Not oxygenated and deoxygenated blood. The colours mean something simpler, and once you know it the screen makes sense.

The near-universal assumption is that red means artery and blue means vein — the way diagrams in biology textbooks are coloured. That is not what the colours mean.

On a colour Doppler image, red means blood moving towards the probe and blue means blood moving away from it. Nothing more. The convention is often remembered as BART — Blue Away, Red Towards. It says nothing about whether a vessel is an artery or a vein, and nothing about oxygen.

The proof is easy to demonstrate and is the moment it usually clicks: turn the probe around and the colours swap. The same vessel, the same blood, flowing the same way — now the other colour, because its direction relative to the probe has reversed.

Why It Works

How Doppler turns movement into colour

You already know the effect from traffic. A siren rises in pitch as an ambulance approaches and drops as it passes — the sound waves are compressed coming towards you and stretched going away. That is the Doppler effect.

Ultrasound does the same with blood. The probe emits sound, red blood cells reflect it back, and cells moving towards the probe return it at a slightly higher frequency while cells moving away return it lower. The scanner measures that shift, decides direction from the sign of it, and paints the result onto the greyscale image — red one way, blue the other.

Brightness carries speed. A vivid, near-white shade means faster flow; a dark, deep shade means slower. So a bright patch is not a different vessel — it is quicker blood.

One practical consequence: colour is angle-dependent. Blood flowing exactly across the beam produces little or no colour even at normal speed, which is why sonographers angle the probe rather than holding it square. An absent colour signal is a prompt to change approach, not automatically a finding.

The Types

Colour, power and spectral Doppler

Colour Doppler is what produces the red and blue overlay — a quick map of where flow is and which way it goes.

Power Doppler uses a single colour, usually orange, and shows the presence and strength of flow without direction. It is more sensitive to slow flow, which makes it useful for detecting blood supply within a small lesion — whether a lump has its own vessels, for instance.

Spectral or pulsed-wave Doppler produces the waveform trace along the bottom of the screen rather than a colour. That graph shows speed over time, and its shape carries diagnostic information: the sharp peaks of arterial flow, the steadier band of venous flow, and the specific patterns that indicate a narrowing upstream. Measurements taken from it — peak systolic velocity, resistance indices — are what a vascular report is built on.

What It Is For

Where Doppler changes the answer

  • Deep vein thrombosis — whether a leg vein contains clot, and whether it compresses normally
  • Carotid artery narrowing — velocity measurements grade how tight a narrowing is
  • Varicose veins — identifying valves allowing blood to flow backwards
  • Testicular and ovarian torsion — absent flow is a critical, time-sensitive finding
  • Pregnancy — umbilical artery flow as part of assessing fetal wellbeing
  • Characterising lumps — whether a lesion has its own blood supply, which contributes to how it is assessed

Safety

Is Doppler safe, including in pregnancy?

Diagnostic ultrasound, including Doppler, has no known harmful effects at diagnostic settings. Doppler does deposit more energy than standard greyscale imaging, which is why professional guidance is to use it purposefully rather than routinely — particularly in early pregnancy, where it is applied when there is a clinical reason rather than to produce an impressive-looking image.

What do red and blue mean on an ultrasound?
